A Study on Health Care Expenditure for the Old Age Group in Korean Public Healthcare System

Abstract
The purpose of this study was to examine health care expenditure of the age group of 65 years and older and to analyze the factors associated with the expenditure. The study was conducted with data on three programs in the Korean Public Health Care System: Korean National Health Insurance(NHI), Medical Aid and Long-term Care Insurance, 2003-2009. The results are consistent with previous literature that the expenditure has been increased rapidly, accounting for 31.4% of the total treatment expenditure of NHI in 2009. The increase rate reaches at 24% in 2009, which is the highest rate since 2003. The factors include the number of older population, levels of income (eligible program types), health care utilization types (in-patient/ out-patient/ pharmaceutic), and related policy changes. The increasing number of older population contributes 16%~35% to increase the total treatment expenditure for the old age group, depending on the utilization and program types. The result shows that the changes in health care expenditure of older Medical Aid¡¯s beneficiaries, different from that of older NHI beneficiaries, is not relevant to the economic situation. Since older persons are likely to suffer from chronic diseases, it is recommended that the individual¡¯s designated physician(clinic) program be implemented and the continuum of care be improved for effective chronic disease management.
